Out of Network Insurance

We do not provide billing for Out of Network services but will provide you with a monthly Super Bill for you to submit to your insurance for reimbursement dependent on your policy.

*We do provide courtesy billing for clients with UMR.

Oregon Health Plan

As of February 2026 all Oregon Health Plan inquiries will be routed though Alive Counseling Clinic. Visit their website (here).

Good Faith Estimate

In compliance with the No Surprises Act, you’re entitled to a Good Faith Estimate explaining how much your medical care with cost. If you are not using insurance, you have the right to receive an estimate of how much your medical care will cost (including medical items and services). You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency items or services including medical test, prescription drugs, equipment, and hospital fees.

You can ask your health provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is a lease $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure you keep a copy of your Good Faith Estimate for your records.
